Healthy Transitions – A Medical Referral Program

Healthy Transitions program is designed to improve your overall health by preventing and treating numerous diseases, including diabetes, hypertension, heart disease, and obesity. The aim of this program is to assist individuals with chronic conditions in establishing a consistent exercise routine of 2-3 times each week. Upon qualifying for this program, you will receive a complimentary two-week guest membership.

Following a comprehensive evaluation of your health and movement screen, functional movement screen, and a short questionnaire, our skilled fitness team will pair you with a small group training program tailored to your specific needs. Additionally, you will have access to a variety of activities that will enhance your experience and support your wellness goals.
